Note:

  1. Upfront payment is required for training. When payment is made, please ensure payment receipt is sent to accountsau@medtechglobal.com with the relevant references. E.g. invoice number, customer no./ customer name.
  2. All travel and Accommodation Fees will be on-charged at cost, including travel time if onsite training is required.
  3. This booking form is not confirmed until our Training Consultant confirms on the training date and an invoice is sent from our Accounts Team.
