This form is for home owners who have already installed a solar PV system to apply for a Solar Homes Package Solar PV rebate. To be eligible for the rebate your installation must meet the eligibility criteria and have had your system installed on or after the 19th of August 2018.
 
Please review the eligibility criteria and have the required documentation ready prior to completing this form:
  • Council Rates Notice for the installation address
  • Australian Tax Office Notice of Assessment for the 2016-17 or 2017-18 financial year for all home owners of the installation address
  • Certificate of Electrical Safety
  • PV STC Assignment Form
  • Distributor's Solar Connection Form or Micro Embedded Generator Connection Form if connected to the grid
  • Electrical Work Request Form
  • Receipt including total cost of system installation, any discounts and rebates applied (e.g. STCs), and the amount owed by you as the customer

We will accept scanned or photographed copies of all required documents. After submitting this form you will receive an initial email receipt. Our rebates team will assess your application over the coming weeks and if your application is complete and compliant with the program guidelines, you will receive a notification that your application has been approved for payment.
