By completing and submitting this form your business will be added to the On-time Payment Small Business Register, which will assist the Queensland Government to pay you faster.

Collection notice

The information is being collected by the Department of Employment, Small Business and Training (DESBT) to enable small businesses to be paid within 20 calendar days.

Information will be retained on the DESBT Customer Relationships Management (CRM) database and be used to update supplier registers and finance system databases across all Queensland Government agencies. The information will enable agencies to identify you as a small business, and pay invoices from your business within 20 calendar days.

You must agree to the following terms and conditions before submitting this form:

  • The applicant of this form must be authorised to make this registration on behalf of the named business.
  • The number of employees listed in the above form is less than 20 employees, the definition of a "small business" as defined in the On-time Payment Policy.
  • The information provided in this Claim Form is complete, true and correct.
  • I understand that providing incomplete, false or misleading information on this registration form may result in my business being considered an ineligible business under the On-time Payment Policy.
  • I understand that I must advise the Department of Employment, Small Business and Training should my circumstances materially change, including in the event that my business no longer complies with the small business definition under the On-time Payment Policy.
  • I acknowledge and consent to the information, including personal information, contained in this registration form being used for the purposes of registering my business in accordance with the Queensland Government's On-time Payment Policy. I further acknowledge and consent to the disclosure of the information, including personal information, contained in this registration form by means of inclusion in supplier registers and finance system databases and being made available to other Queensland Government agencies for the purposes of enabling payment of my business' invoices within 20 calendar days.
